Key Review Excerpts

Amazing PT, OT, & speech therapy totally offset by poor nursing care. Aids don't follow OT's written plan of care, do not do routine nursing care unless family member is there, and many are extremely rough with the patients.

Rehab patient family member · 2017☆☆☆☆

My mom was at clearview for a year after a tbi and stroke. She was treated so well and so were we! Nothing bad to say about it, lovely staff and facility.

Long-term resident's family · 2022★★★★★

There was no communication between the staff. I had questions and 3 out of 4 employees said, 'I don't know'. Only 1 said ,'I will get you answers.' They did not comb her hair out brush her teeth.

Clean record — no deficiencies

Clearview Brain Injury Center shows a concerning pattern of recurring safety and emergency preparedness issues across three consecutive surveys, with deficiencies in emergency planning, fire safety systems, and electrical systems appearing multiple times. While all violations have been corrected according to facility reports, the repeated nature of safety-related problems—particularly around emergency preparedness, fire alarm/sprinkler systems, and electrical infrastructure—suggests ongoing operational challenges that families should discuss during visits.
