What this means for your family
This facility is highly regarded for its compassionate nursing staff and effective rehabilitation services, making it a strong candidate for post-acute care. While the feedback is overwhelmingly positive, we recommend scheduling an in-person tour to observe the staff-to-resident interaction firsthand, as the majority of reviews are brief and lack deep operational detail.

Key Review Excerpts
“My father fell and broke his femur. He went into THE CENTER ON AGING AND HEALTH after his surgery for rehab. The staff was so professional and caring to him. Soon he felt at ease. Any questions that we had was quickly answered. PT and OT worked very hard with him and he was soon walking with a walker and help from the PT staff.”
“I had the opportunity to test a new service being implemented by COAH called Doxyme webchat. It is videoconferencing that allows remote family members to have Care meetings and visit with their loved ones. I talk to my mother frequently on the phone, but being able to see her (and for her to see me as well) was wonderful.”
“The lady that shaves and bathes the people is amazing, not only is she good at what she does she has a personality plus. Always speaks to you and has a smile for everyone. Thank you very much for keeping my brother so neat and clean.”

This facility has recurring issues with care planning across multiple surveys, with families filing at least one complaint that triggered an inspection in 2025. The most frequent problems involve developing complete care plans for residents, electrical and safety systems, and emergency preparedness protocols. While all deficiencies show correction dates, the repeated care planning violations and recent complaint suggest ongoing challenges with ensuring residents receive properly coordinated care.
