What this means for your family

While recent leadership changes have garnered praise for responsiveness, the facility continues to face serious, recurring reports of poor sanitation and roach infestations. We strongly recommend an unannounced tour of the specific wing your loved one would occupy, paying close attention to room cleanliness and the presence of pests.

Key Review Excerpts

My father was a patient at Cedar Hill Nursing and Rehab Center for 3 months prior to his passing in the facility. I truly believe that Sitoria the DON and the CNA team did the best that they could while handling my fathers care.

Long-term resident's family · 2024★★★★★

Families filed reports that triggered inspections finding serious concerns about abuse protection and accident prevention, though the facility corrected these issues. The facility has recurring problems with environmental safety (equipment maintenance, pest control), fire safety systems, and facility cleanliness that appear across multiple surveys from 2021 to 2024. All cited deficiencies show correction dates, but the pattern of repeated environmental and safety violations suggests ongoing operational challenges families should discuss during visits.
