The facility failed to ensure that one of three sampled staff members had completed an examination and screening for the presence of controlled substances upon hire. The personnel record for Staff C, a personal care aide, lacked documentation of a completed controlled substance screening.

The facility failed to ensure one of three sampled residents received a tuberculosis test upon admission. Specifically, Resident #2 had no documentation of a second TB skin test or a TB skin test performed upon admission to the current facility.

The facility failed to ensure that Resident #2 received a required tuberculosis (TB) test upon admission. Records showed a TB skin test from 2015 but no documentation of a second test or a test performed upon the resident's admission on 01/25/20.

The facility failed to implement CDC and NC DHHS guidance during the COVID-19 pandemic. Specifically, staff members were not wearing face masks and the facility failed to conduct required daily symptom screenings for residents and staff.

The facility failed to implement CDC, NC DHHS, and local health department guidance regarding COVID-19 protections. Specifically, the facility did not adequately perform screening of visitors, staff, and residents for symptoms, nor did they ensure proper use of personal protective equipment (PPE) by staff.

The facility failed to ensure that all current orders for medications or treatments, including orders for self-administration, are reviewed and signed by the physician at least every six months. The medication aide was unaware that Resident #2's medication orders needed to be updated following his move.

The facility failed to comply with a Suspension of Admissions issued on 12/19/19 by admitting one additional resident (#2) who was not a resident prior to the notification. The owner moved the resident from a sister facility to comply with code enforcement requirements but did not consider the move a new admission.

The facility failed to ensure that the examining date on Resident #2's FL2 form was no more than 90 days prior to admission. A review of the resident's record showed an FL2 dated 08/08/19, while the resident had been admitted to the facility on 06/11/15.

The facility failed to comply with a Suspension of Admissions order issued on 12/19/19. During the survey, inspectors identified an additional resident living at the facility who was not accounted for by management, violating the restriction on admitting new residents during the suspension period.
