The facility failed to administer medications as ordered by a licensed prescribing practitioner for one resident. Specifically, the owner was administering an anti-anxiety medication (Trazodone) as needed rather than the ordered three times daily, and failed to document medication refusals. Additionally, a laxative (Metamucil) was not administered as ordered due to a failure to clarify a discrepancy between the physician's order and the MAR.

The facility failed to administer medications in accordance with physician orders for one resident. Specifically, the facility failed to follow the prescribed dosage and frequency for Trazodone and a laxative, as evidenced by discrepancies between the physician's orders and the medication administration records and physical medication counts.

The facility failed to implement required infection prevention and control protocols during the COVID-19 pandemic. Specifically, staff members were not wearing face masks and the facility was not performing necessary daily symptom screenings for residents and staff.

The facility failed to implement recommended CDC and NC DHHS guidance regarding infection prevention. Specifically, the Medication Aide/Owner was observed not wearing a face mask upon entering the facility, despite policies requiring face coverings. Additionally, the facility's infection control policy was not consistently applied to all staff members.

The facility failed to implement CDC and NC DHHS guidelines regarding COVID-19 infection control. Specifically, the facility did not adequately screen visitors, staff, and residents for symptoms, nor did they ensure proper temperature documentation or consistent use of face masks to reduce the risk of transmission.

The facility failed to ensure that 2 of 3 sampled staff members had no substantiated findings listed on the North Carolina Health Care Personnel Registry (HCPR) upon hire. Specifically, the personnel record for one staff member lacked a documented hire date and evidence of a completed HCPR check.
