Additional Finding: Home Manager LaJuana Davis admitted to kicking Resident A in the hip/face area during an altercation; a resident witness reported the manager also grabbed the resident by his hood.

Medication error: Resident A’s medications were incorrectly logged as administered on 04/31/2023 (a non-existent date), and Lithobid was not initialed as administered on 04/30/2023.
Bathrooms were equipped with locking against egress hardware.
Staff member Tre’Von Davis did not have an annual health review on file for the year 2022.
The rear egress was blocked due to kitchen remodeling, and large amounts of wood and debris were stored near the rear exit and side of the home.
REPEAT VIOLATION: Hot water temperature was found to be 160 degrees Fahrenheit, exceeding the allowed range.
