Rehab patient · 2025★★★★★

While there on Sunday April 27th, the other resident was sitting in his wheelchair in a pond of urine that was running all over the floor. They had a towel on the floor under the wheelchair that was clearly soaked!

Family member · 2025☆☆☆☆

My bedside kimode overflowed causing me to put my hand in it. The food was horrific and I lost 30 lbs. My clothes were left in the laundry, but I was welcome to go through lost and found.

Care Manor has concerning patterns of deficiencies with families filing complaint reports about infection control and resident rights issues. The facility repeatedly struggles with care planning and resident assessments across multiple surveys, along with medication management and infection control problems. While all violations show correction dates, the recurring nature of care planning deficiencies suggests ongoing challenges in providing consistent, individualized resident care.
