What this means for your family

While some families report success with short-term rehab, the recurring, severe reports of neglect and communication failures are deeply concerning. If you consider this facility, we strongly recommend unannounced visits, verifying staffing ratios, and speaking directly with current residents' families rather than relying on administrative tours.

Brookside Healthcare Center receives highly polarized feedback, with some families praising the rehab therapy and specific staff members, while others report severe neglect, poor communication, and hygiene issues. Critical concerns include understaffing, difficulty reaching administration by phone, and reports of residents being left in unsafe or unsanitary conditions. Potential families should be aware of the stark contrast between positive experiences with individual staff and recurring reports of systemic failures.

Key Review Excerpts

He was being called "Carl" and other names. The name tag on the room entrance said a different name entirely. His dentures had ten days worth of dried glue in them, had never been cleaned, and didn't even fit in his mouth correctly.

Memory care family member · 2026☆☆☆☆

Within 7 hours, she had to be rushed back to Christ because she couldn't breathe and no one would come to help her. If it weren't for another patient trying to reach 911, she wouldn't be alive right now.

Long-term resident's family · 2023☆☆☆☆

The therapy department was very attentive and really pushed him. He’s currently back home in his own apartment doing well and if we ever need a placement again, Brookside would be the choice all over again.

Well below state avg (10.1)

Brookside Healthcare Center shows a pattern of recurring deficiencies across fire safety systems, medication management, and nutritional services that have persisted across multiple surveys from 2019 to 2025. The facility has had issues with fire alarm maintenance, emergency equipment, dietary planning, and pharmacy oversight that appear repeatedly despite corrections. While all deficiencies show correction dates, the recurring nature suggests ongoing challenges with maintaining compliance standards.
