What this means for your family

While some families report positive outcomes with specific staff members and rehab services, the facility faces severe and recurring complaints regarding basic hygiene and responsiveness. If you are considering this facility, we strongly recommend an unannounced visit to check the cleanliness of the environment and to observe staff interaction with residents in common areas.

Brookshire Healthcare Center receives highly polarized feedback, with many families reporting severe neglect, poor hygiene, and chronic understaffing. While some reviewers praise individual staff members for their kindness, these positive experiences are frequently overshadowed by recurring reports of residents left in soiled conditions, unresponsive nursing staff, and a facility-wide odor of urine. Families should be aware that the facility's reputation for basic care and communication is significantly inconsistent.

Key Review Excerpts

My mother in law was transferred here after a hip replacement and the care was AWFUL. I am a CNA who has worked in an under staffed facility numerous times, and one thing is… I’m always going to take care of my patients. My patients will NEVER have a sopping wet 5 pound brief on that hadn’t been checked or changed in 12 hours.

Family member of rehab patient · 2024☆☆☆☆

The only negatives are the food and the building itself which is run down. But the caring staff makes those things seem less important.

Family member of rehab patient · 2025★★★★

Brookshire's got the same issues that you'll find in any other for-profit facility like worker shortages and overcrowding, but I would recommend them for short-term stays. My father has advanced dementia, and he was treated with respect, was well fed and cleaned.

Brookshire Healthcare Center shows a pattern of fire safety and building maintenance issues that have persisted across multiple surveys, including problems with sprinkler systems, smoke barriers, and heating/ventilation systems. The facility also had recurring deficiencies in care planning and isolated incidents involving infection control and reporting requirements. While all deficiencies show correction dates, the repeated fire safety violations across 2018-2022 surveys suggest ongoing infrastructure challenges that families should discuss during visits.
