Water temperature was measured at 121 degrees Fahrenheit in one resident bathroom and at 125 degrees Fahrenheit in the general bathroom, exceeding the allowed range of 105-120 degrees Fahrenheit.
Only two fire drills were practiced and/or documented during the second quarter of 2025, failing to meet the requirement for quarterly practice per shift.
One resident refrigerator and the medication refrigerator were not equipped with a thermometer.
