Key Review Excerpts

What really makes this place shine is the therapy department. They were amazing to work with and clear on setting realistic goals that were enough to be achievable. Highly recommend for the therapy department alone!

Family of a short-term rehab resident · Feb 2026★★★★★

The staff is attentive, facility is clean. My mother suddenly passed away early morning June 8, I received a call from Angie her nurse, she was compassionate and in tears herself. I want to thank her from the bottom of my heart it meant the world to me knowing that mom had someone present that cared for her.

Family of a deceased resident · Jun 2025★★★★★

The physical therapy at Berkshire is great! Jeff does a great job! I'm sure all is them are great I just don't know their names. Jeff is the one who works with me husband the most. Cassie and Michelle are 2 of the best nurses! They are very kind and compassionate. I didn't give 5 stars because the food is really bad.

3 complaint-triggered

Multiple families have filed complaints about this facility, triggering federal investigations that found serious issues with care planning, medication management, and resident safety. Recurring problems include inadequate care plans, medication errors exceeding acceptable rates, safety hazards, and violations of resident rights including improper discharge procedures and restricted visitation. While the facility has corrected each cited deficiency, the pattern of 38 violations across five surveys, including repeated medication and care planning failures, suggests ongoing quality concerns families should carefully evaluate.
