What this means for your family

While the facility is frequently praised for its cleanliness and some staff members are described as compassionate, there is a concerning pattern of reports regarding patient neglect, fall safety, and dietary errors. We strongly recommend that you visit unannounced, observe the level of assistance provided during meal times, and ask management specifically how they address patient fall prevention and dietary compliance.

Key Review Excerpts

My dad was here for rehab for 3 weeks earlier this year. While there they continuously gave him the wrong diet, he needed gallbladder surgery and could not eat fatty foods yet they would serve him these things even after we told them multiple times.

Rehab patient's daughter · 2024☆☆☆☆

So despite the fact that I've been checking in on my 93 year old dad twice a day since he's been there... they called me tonight to tell me he slid out of bed into the floor. His room is literally three doors down from the nursing desk.

Long-term resident's daughter · 2025★★☆☆☆

Cindy Hollis is a Awesome Administrator she deserves a raise.. she came in and really started making some changes for the better been here as a resident for almost 4 years now and the staff had changed through the time here, but they always show compassion to us!

Clean record — no deficiencies

Families filed complaints that triggered a 2024 survey revealing deficiencies in care planning and respiratory care, both areas that had previously been cited in 2020. The facility shows recurring issues with care planning, resident safety, and building systems across multiple surveys. While all deficiencies have correction dates, the pattern of repeated problems in critical care areas warrants careful consideration during any visit.
