This Statement of Deficiencies (SOD), supersedes the SOD sent on March 5, 2025. The following deficiencies were found during the on-site compliance inspection and investigation of complaints AZ00207250, AZ00197332. and AZ00191900 conducted on February 3, 2025.

Based on observation and interview, the manager failed to ensure that poisonous or toxic materials stored by the assisted living facility were maintained in labeled containers in a locked area separate from food preparation and storage, dining area and medications are inaccessible to residents. The deficient practice posed a risk to the physical health and safety of a resident. Findings include: 1. During the environmental inspection, the Compliance Officers (CO) observed a bottle of Clorox Disinfecting Wipes, Lemon Fabuloso Cleaner, and Kirkland Signature Dishwasher PACS inside an unlocked kitchen sink cabinet next to multiple open boxes of coffee. 2. During the environmental inspection, the COs observed Lysol Toilet Bowl Cleaner and Clorox Disinfecting Wipes in two separate resident bathrooms. Both bathrooms were equipped with locks on the cabinet doors but neither were secured. 2. In an interview, E1 acknowledged that the poisons or toxins mixed with the coffees in the kitchen and in the residents bathrooms were in violation of the rules.
