What this means for your family

This facility's physical therapy department is frequently praised for helping patients regain mobility, making it a potential option for short-term rehab. However, families must be extremely cautious regarding medication management and facility cleanliness; we strongly recommend conducting an unannounced visit to inspect the rooms and verifying the facility's current medication administration protocols before admission.

Key Review Excerpts

The fantastic care and great therapist there got her up and walking in three weeks. Between the great nursing staff and therapist both physical and occupational they did an outstanding job.

Long-term resident's family · 2019★★★★★

The nursing and aids are some of the kindest people I have ever met. The kitchen does well with what they are given (sorry guys, I know it's rough.) The facility itself is very clean and maintenance acts quickly to rectify issues.

Long-term resident's family · 2025★★★★

I honestly do not even want to give this place a 1 star rating. This was the MOST disgusting place I have ever stayed. My first night there I slept on a mattress of someone else’s piss.

Families have filed multiple complaints resulting in deficiencies related to abuse reporting failures, safety hazards, and inadequate staffing. The facility shows recurring issues with fire safety systems, medication management, and resident care quality across multiple surveys from 2023-2025. While all deficiencies show correction dates, the persistent pattern of problems in critical areas like safety, pharmacy services, and care planning suggests ongoing operational challenges families should carefully consider.
