The State relicensure survey was conducted on October 5, 2023. The following deficiency was cited:
Based on review of employee roster and staff interview, the facility failed to provide evidence of orientation and in-service education training on abuse and neglect for 2 direct care staff (# 1 and # 4) Findings include: A review of the personnel file of the Director of Nursing (staff #1) revealed a hire date of June 14, 2006. A review of the personnel file of the Administrator (staff #4) revealed a hire date of January 7, 2013. However, further review of the personnel file of both staff #1 and #4 revealed no evidence that staffs #1 and #4 had orientation and in-service education and training on abuse and neglect. In an interview with the assistant administrator (staff #6) conducted on October 5, 2023, at 11:31 a.m., he stated that staff #1 and #4 does not have orientation and in-service education because staff #1 and #4 does not work directly with client. Review of the facility policies and procedure revealed that newly hired employees are required to have orientation to the Nursing Supported within 14 days upon date of hire. It also included that employees are required to complete a new member orientation every 6 months, to review any changes or updates to the member or the department.
