Key Review Excerpts

My mother was sent there from MidMichigan Hospital. She has a history of filling up with fluid, due to heart failure, sepsis shock... I had asked when she would be able to see a doctor. I was told 'Ohh with in the first 24hrs.' Awesome, no problem. Thursday night rolls around, she still has not seen a doctor.

Long-term resident's family · 2021☆☆☆☆

There are some really kind professional and skilled people who work here. And there is also a tremendous inconsistency. Meals arrive late and in error. Medications arrive late. Aides, nurses and PT - OT staff are in dire need of retraining in HIPAA policies.

Family member · 2023☆☆☆☆

My father was neglected and mistreated here. He came here for short term rehab after a stroke, lost his left side, had two brain surgeries. He needed full assistance on mostly everything. I heard the staff complaining in the hallway about cutting up his food.

7 complaint-triggered

This facility shows persistent issues across multiple areas, with families filing four complaint reports that triggered federal investigations. The most recurring problems involve care planning and assessments, medication management, and infection control, with these same issues appearing across all three surveys from 2022 to 2025. While the facility has corrected each deficiency when cited, the pattern of repeated violations in core care areas suggests ongoing systemic challenges that families should carefully consider.
