What this means for your family

While some families report a well-maintained facility and professional administrative support, the recent, severe allegations of neglect and failure to provide physical therapy are deeply concerning. We strongly advise families to conduct unannounced visits and closely monitor the care provided to loved ones, especially regarding hygiene and therapy schedules.

Key Review Excerpts

The facility is well-kept and she reported that the food was reasonably good. Most of the staff were quite friendly and helpful, although there were a few that were somewhat less friendly.

Long-term resident's family · 2025★★★★

My father left here on a stretcher in an altered state!!! The staff neglected him !! I have a list of things, but I will keep this review short as possible. The facility was not doing physical therapy with him; the facility was not clean.

Long-term resident's family · 2023☆☆☆☆

My dad would have died if it wasn't for the vigilance of my family. He was there for a short period of time (11 days) and became extremely ill. They didn't "attempt" to do their jobs unless we were present.

This facility has a concerning pattern with 10 of 17 deficiencies triggered by family complaints, indicating ongoing issues that prompted external reports. The most recurring problems involve resident rights violations (accommodating preferences, maintaining dignity, handling grievances), care planning and assessments, and quality of care issues including safety hazards and medication management.
