What this means for your family

While the recent renovations and new management have clearly improved the facility's physical environment and dining, the history of serious neglect and staffing issues remains a concern. When touring, we strongly recommend you observe the response time to call lights and ask specifically about the current nurse-to-patient ratios on weekends and night shifts.

Aviata at the Harbor (formerly Consulate of Safety Harbor) has undergone significant changes, including a recent remodel and leadership transition, which have led to a sharp divide in reviewer experiences. While many recent reviews praise the facility's cleanliness, improved dining, and attentive rehab staff, a persistent pattern of critical complaints regarding neglect, poor communication, and staffing shortages remains a serious concern for families. Prospective families should weigh the positive reports of the facility's physical transformation against the recurring, severe allegations of inadequate patient care.

Questions for Your Tour

  • 1I noticed your team is very active in responding to online feedback; what is the best way for families to communicate directly with management if they have concerns about their loved one's care?
  • 2With the recent facility renovations, what specific steps are you taking to maintain high standards of cleanliness and pest control throughout the building?
  • 3Could you walk me through your current process for medication administration and how you ensure accuracy for residents with complex health needs?
  • 4When a resident uses their call light, what is your standard protocol for response times, and how do you monitor this during busier shifts?
  • 5Given the focus on physical and occupational therapy here, how are these programs integrated into the daily routine for residents who need extra support?
  • 6How do you keep families updated on changes in a resident's health status, especially when we are trying to reach someone by phone during busy hours?

Key Review Excerpts

The recent remodel has made a huge difference. The building looks beautiful, modern, and comfortable. It is always clean and well taken care of. The staff are friendly and caring, and the food is some of the best I’ve seen.

Family member · 2025★★★★★

The amount of negligence and cutting corners at this place is unbelievable. The food portions don't match what is on the tickets. You have to fight to get therapy or doctors appointments. They don't clean wounds properly, if they clean them at all.

Family member · 2024☆☆☆☆

My mother was placed here Friday night I took her out 15 minutes after arriving. This place was deplorable. Human feces on bathroom walls, no hand soap sink was dirty, hole in wall under ac unit floor was sticky and smelled like urine.

This facility shows recurring problems across multiple surveys, with families filing complaint reports about cleanliness and food quality. The most frequent issues involve medication management (including high error rates), food service and nutrition, and fire safety systems. While all deficiencies were reportedly corrected, the pattern of repeated violations in medication management, dietary services, and safety systems across four surveys suggests ongoing operational challenges that families should carefully consider.
