Questions for Your Tour

  • 1Given the recent feedback regarding staffing levels, what specific steps is the facility taking to ensure that residents receive timely assistance with daily hygiene and toileting needs?
  • 2I noticed the CMS staffing rating is currently lower than average; how does your leadership team ensure that your nursing staff is adequately supported so they can provide consistent, high-quality care?
  • 3Medication management is a priority for us; could you walk us through your current process for administering medications and how you ensure accuracy and timeliness for each resident?
  • 4We read wonderful things about your rehabilitation therapy team; how do they collaborate with the nursing staff to ensure a resident's progress is maintained throughout the rest of the day?
  • 5With 116 residents on campus, how do you balance the need for personalized attention with the demands of a larger facility to ensure no one feels overlooked?
  • 6We are very interested in the social activities mentioned by others; could you describe a typical week of programming and how you encourage residents with different mobility levels to participate?

Key Review Excerpts

The nursing, therapy and Dining services were wonderful and accommodating. Thank you for easing the stress of being a caregiver and supportive

Long-term resident's family · 2024★★★★★

The rehab part is great while the care she received was awful. The answer to everything was , "we are short staffed ". They give showers twice a week if your lucky., and if you get one the water is cold . The call light time is whenever they want 2-6 hour wait time for meds or to go to the bathroom.

Rehab patient's family · 2026☆☆☆☆

My father was at Oakwood Village for the last month-and-a-half of his life. The care was compassionate. ALL of the staff who interacted with dad were wonderful and kind. The facility was very clean.

15 complaint-triggered

Aventura at Oakwood Village has significant inspection issues with families filing 15 complaints triggering federal investigations. The facility repeatedly struggles with resident safety (accident prevention appearing in multiple complaint surveys), medication management (including error rates exceeding 5%), and abuse/neglect reporting procedures. While deficiencies show correction dates, the pattern of recurring safety and medication problems across multiple years, combined with frequent family complaints, suggests ongoing operational challenges that warrant careful consideration before placement.
