Bedside assistive devices for Residents A and B were insecure and loose; service plans lacked detail regarding staff responsibilities for inspection, maintenance, and resident summoning.
Staff administered Metoprolol to Resident A despite vital signs (blood pressure and heart rate) falling below the prescribed hold parameters.
Inadequate and indiscernable air flow/ventilation in bathing/toilet facilities (rooms 309 and 419), the third-floor public restroom, and the laundry room.
