The following deficiencies were found during the on-site compliance inspection and investigation of complaint 00136629, 00104383, and 00108591 conducted on July 28, 2025.
Based on record review and interview, the manager failed to ensure that two of eight sampled residents provided evidence of freedom from infectious tuberculosis before or within seven calendar days after the resident’s date of occupancy. The deficient practice posed a TB exposure risk to residents. Findings include: 1. A review of R1 and R8's medical records revealed, neither resident provided a TB screening assessment. 2. In an interview, E1 acknowledged R1 and R8 did not provide evidence of freedom from infectious tuberculosis before or within seven calendar days after the resident’s date of occupancy.
Based on observation and interview, the manager failed to ensure medical records were maintained for R4. The deficient practice posed a risk as the required information could not be verified. Findings include: 1. During the environmental inspection, the Compliance Officer was informed by E1, that R4 was no longer a resident of the facility. 2. The Compliance Officer requested to review the medical record for R4. However, the medical record for R4 was not provided for review. 3. In an interview, E1 acknowledged the medical records for R4 were not maintained as required.

The deficient practice posed a risk as the required information could not be verified. Findings include: 1. During the environmental inspection, the Compliance Officer was informed by E1, that R4 was no longer a resident of the facility. 2. The Compliance Officer requested to review the Medical Administration Record (MAR) for R4. 3. The Medical Administration Record for R4 was not provided for review. 4. In an interview, E1 acknowledged a medication administered to a resident was not documented in the resident' s medical record.
