A relicensure survey was completed on 10/17/23. A deficiency was cited. Based on observation and interview, the residence failed to ensure each common area bathroom had liquid soap and paper towels or hand drying devices available at all times, affecting nine current residents. Findings include: An environmental tour on 10/17/23 at approximately 9:00 a.m., revealed there was no hand soap or hand drying devices that were available to residents at all times in each of the four common area bathrooms. An environmental tour lead by Staff #3 at 9:06 a.m., revealed there was a high shelf in the far north bathroom that contained paper towels and a locked closet that required a code to unlock that contained hand soap.On 10/17/23 at 8:58 a.m., Staff #3 stated there was no hand soap or hand drying devices in any of the four common area bathrooms because Resident #1 would grab anything she could see and play with it. Staff #3 stated the paper towels were kept up high in the north bathroom so Resident #1 could not grab it, and the hand soap was kept locked in the bathroom so Resident #1 was unable to access it. Staff #3 acknowledged no other residents knew the code to get hand soap either. On 10/17/23 at 9:05 a.m., Resident #2 stated that Resident #1 would take the hand soap and play with it. Resident #2 stated he had his own paper towels and soap in his bedroom; however, was frustrated it was not in the common bathrooms such as the one .. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised it must review and maintain the following processes in accordance with existing program regulations found at 6 CCR 1011-1, Chapter 7.14.29 All prescribed and PRN medications shall be listed and recorded on a medication administration record (MAR) which contains the name and date of birth of the resident, the resident ' s room location, any known allergies, and the name and telephone number of the resident ' s authorized practitioner. (C) Each qualified medication administration person, nurse, or practitioner shall accurately document each medication administration or monitoring event at the time the event is completed for each resident.
