FACILITY EVALUATION REPORT

This report details a complaint investigation conducted on the facility. All three allegations—staff causing injury, staff training, and hygiene care—were reviewed and subsequently deemed UNSUBSTANTIATED based on the evidence gathered during the investigation.

The inspection was a Case Management visit conducted on August 14, 2025. The primary deficiency cited relates to the failure to secure proper criminal record clearances for an individual working at the facility. This violation was classified as Type A due to the immediate health and safety risk posed to residents in care.

The facility was visited unannounced for a case management follow-up regarding an incident reported on 07/30/2025. The Licensing Program Analyst interviewed staff and reviewed documentation related to the incident. No deficiencies were observed during this visit.

The facility underwent an unannounced annual/required visit. The inspection covered common areas, resident rooms, activity rooms, and essential services like medication storage and kitchen areas. No deficiencies were cited during this visit, and the report confirms that all inspected areas were found to be in good order.

The facility underwent an unannounced Case Management visit. The Licensing Program Analyst interviewed four residents and reviewed their files, noting that all required documentation was in compliance and that medications were compliant. No deficiencies were cited during this visit.
