What this means for your family

While the admissions team is frequently praised for their helpfulness, the facility suffers from persistent, serious complaints regarding cleanliness and missing personal items. We strongly recommend conducting an unannounced visit to inspect the living conditions and asking specifically about their process for tracking resident belongings and ensuring consistent communication with family members.

Key Review Excerpts

The bathrooms and shower rooms look like they haven’t been cleaned, fixed, or cared about in forever. Toilets stay clogged for days, dirty towels and sheets sit on the floor, tiles are missing, and there are straight-up holes in the walls.

Family member · 2025☆☆☆☆

The staff has taken her and addressed her needs with care. I appreciate the staff and all who have helped her and worked with our family. The building is an older facility. So far I haven't had to complain to anyone over cleanliness or safety issues.

Family member · 2025★★★★★

Less than a week after my aunt's arrival at the facility, EVERY piece of clothing that she entered Arrowhead Health and Rehab with was stolen. It was stolen despite a note placed stating that the family would do laundry and notifying the staff of such.

17deficiencies
Well above state avg (6.1)
21 complaint-triggered

A concerning pattern emerges with 21 deficiencies triggered by family complaints, indicating ongoing resident and family dissatisfaction. The facility repeatedly struggles with resident rights, care planning, and quality of care issues across all three years, suggesting persistent problems rather than isolated incidents. While the facility reports correcting all deficiencies, the recurrence of similar issues—particularly around respecting resident dignity, developing proper care plans, and maintaining basic care standards—raises questions about lasting improvements.
