The following deficiencies were found during the on-site compliance inspection and investigation of complaint 0000124011 conducted on August 6, 2025.
Based on documentation review and interview, the manager failed to ensure a disaster drill for employees was conducted on each shift at least once every three months and documented. The deficient practice posed a risk if employees were unable to implement a disaster plan. Findings include: 1. A documentation review of the facility’s "Employee Disaster Drill Sheet” revealed that the last disaster drill was completed on January 14, 2025. There was a blue sticky note with writing that read, "Next Drill 4/14/25". 2. A documentation review of the facility's Policies and Procedures manual titled, "Fire Safety and Disaster Plan" policy, item number three, stated “Staff disaster drills will be completed every three months.” 3. In an interview, E2 acknowledged the manager did not ensure a disaster drill for employees was conducted on each shift at least once every three months and documented.

The deficient practice posed a risk if employees were unable to implement a disaster plan. Findings include: 1. A documentation review of the facility’s "Evacuation Drill” revealed that the last evacuation drill was completed on October 16, 2024. There was a orange sticky note with writing that read, "Next Drill 4/16/25". 2. A documentation review of the facility's Policies and Procedures manual titled, "Fire Safety and Disaster Plan" policy, item number four, stated “Employee and resident evacuation drills will be completed every six months.” 3. In an interview, E1 acknowledged the manager did not ensure an evacuation drill for employees and residents was conducted once every six months and documented.
