Key Review Excerpts

The facility is old and had a roof leak in a major hallway with sopping bed pads on the floor to absorb the leak. The ceiling tiles were already molding due to how long the leak occured.

Long-term resident's family · 2024☆☆☆☆

My Father was a patient here for respite care last week. My Mother and I pulled him out after 36 hours. Live cockroaches! One jumped out of his trash can the size of a small car.

Long-term resident's family · 2026☆☆☆☆

The therapy department does a great job at getting people rehabilitated. The Administrator Chandler and Director of Nursing Rebecca does the absolute best job at running the facility.

This facility has 15 deficiencies across four surveys, with families filing four complaint-triggered reports about resident rights, privacy, and staffing transparency. The most recurring issues involve fire safety equipment, building safety features, and food handling practices. While all deficiencies show correction dates, the pattern of complaints and repeated food safety violations in both 2019 and 2023 suggests ongoing operational challenges that warrant careful consideration.
