FACILITY EVALUATION REPORT

The facility underwent an annual Infection Control Inspection on 09/10/2025. This report details a complaint investigation conducted on 5/1/2025 regarding allegations of staff neglecting resident care. The investigation found that there was not a preponderance of evidence to prove the alleged violation occurred, resulting in the allegation being unsubstantiated.

This report details a complaint investigation regarding a facility staff member allegedly leaving a resident in soiled diapers, resulting in skin breakdown. The investigation concluded that there was not a preponderance of evidence to prove the alleged violation, leading to the allegation being deemed UNSUBSTANTIATED.

The annual inspection was conducted on August 12, 2022. The facility was observed to have adequate supplies, proper safety equipment, and general compliance measures in place. One deficiency was noted requiring the posting of updated handwashing signage in shared bathrooms.
