The following deficiencies were found during the on-site compliance inspection conducted on May 13, 2024:
Based on observation and interview, the manager failed to ensure that the facility was free from condition or situation that may cause a resident or other individual to suffer physical injury. The deficient practice posed potential egress dangers to residents. Findings include: 1. The Compliance Officer observed debris and other materials outside which were blocking the exit to the front of the facility on the east side of the facility. 2. In an interview, E1 acknowledged that debris and other materials outside were blocking the exit to the front of the facility on the east side of the facility.
Based on observation and interview, the manager failed to ensure that two oxygen containers were secured. The deficient practice posed a potential explosion or leak of a compressed gas. Findings include: 1. The Compliance Officer observed two upright and unsecured oxygen tanks in R1's room. 2. In an interview, E1 confirmed that there were two upright and unsecured oxygen tanks in R1's room.
Based on observation and interview, the manager failed to ensure that poisonous or toxic materials were stored in a locked area. The deficient practice posed a risk to the physical health and safety of a resident. Findings include: 1. The compliance officer observed the following toxic items in the facility's kitchen: - One can of Cutter Backyard Bug Control bug spray found underneath the kitchen sink and was accessible to residents. - One can of Hot Shot Flying Insect Killer bug spray found underneath the kitchen sink and was accessible to residents. - One bottle of STEM bug spray found underneath the kitchen sink and was accessible to residents. 2. In an interview, E1 acknowledged that the above chemicals were under the kitchen sink unsecured and accessible to residents.
