The following deficiency was found during the on-site abbreviated follow-up inspection conducted on November 20, 2024:

A manager shall ensure that a personnel record for each employee or volunteer:R9-10-806.C.1.a-cCorrected Nov 28, 2024

Based on observation, record review and interview, for three of three employees reviewed, the manager failed to have a personnel record for an employee, as required by this Article. The deficient practice posed a risk to resident health and safety if the facility did not obtain documentation showing an employee met the requirements to provide services for the residents. The deficient practice posed a risk as required information could not be verified for E1, E2, and E3. Findings include: 1. In observation, E1, O1, and E3 came to the facility for the abbreviated inspection. No residents were present or resided at the facility. 2. During an interview, O1 reported the facility changed ownership, and had a new manager and two new caregivers. The Compliance Officer requested to review the personnel records for the new employees. 3. In record review, the facility did not have personnel records on site for the employees; however, were able to obtain documents and print them for review. The Compliance Officer was not provided all required documentation in accordance with R9-10-806.C.1. The records did not include the following: - E1 (hired November 18, 2024) - Documentation of the individual's qualifications, including skills and knowledge applicable to the individual's job duties; the individual's education and experience applicable to the individual's job duties; the individual's completed orientation and in-service education required by policies and procedures; the individual's license or certification, if the individual is required to be licensed or certified in this Article or in policies and procedures; evidence of freedom from infectious tuberculosis, and compliance with A.R.S. \'a7 36-411(C). - E2 (hired November 18, 2024) - Documentation of the individual's qualifications, including skills and knowledge applicable to the individual's job duties; the individual's education and experience applicable to the individual's job duties; the individual's completed orientation and in-service education required by policies and procedures; the individual's license or certification, if the individual is required to be licensed or certified in this Article or in policies and procedures; evidence of freedom from infectious tuberculosis, and compliance with A.R.S. \'a7 36-411(C). - E3 (hired on November 9, 2024) - Compliance with A.R.S. \'a7 36-411(C). 4. During an interview, O1 reported E1 and E2 were affiliated with a management company called Modern Care Homes, and as "caregivers," would provide assisted living services for residents, and E3 was the manager. E1 was assisting with the facility's administration. O1 reported being the new owner, and acknowledged the personnel records for the employees were incomplete and did not include the required documentation.
