Maintenance/Housekeeping issues: worn toilet seat in community bathroom, dirty toilet in resident room #38, caulking around sink in resident room #37 needs replacement, and no mirror/vanity in resident room #37.
No annual assessment plans for 2021 were contained in the files for Resident B, Resident C, and Resident D.
There was no health care appraisal contained in Resident A’s resident file at the time of admission.
