The following deficiencies were found during the on-site abbreviated follow-up inspection, and investigation of complaint #AZ00206580, conducted on February 21, 2024:

Based on record review, and interview, for two of four caregivers reviewed, the manager failed to ensure a caregiver provided documentation of cardiopulmonary resuscitation training (CPR) certification specific to adults which included a demonstration. The deficient practice posed a health and safety risk to residents if caregivers did not have CPR training which included a demonstration of the employee's ability to perform CPR. Findings include: 1. In record review, E3's personnel record included documentation of CPR certification, dated October 9, 2022, from NationalCPRFoundation, which was an online training program, and did not include a demonstration. E4's personnel record included documentation of CPR certification, dated September 27, 2023, from National Health & Safety Association, which is an online training program, and did not include a demonstration. 2. During an interview, the findings were reviewed with E1 and E2, acknowledged CPR training received by E3 and E4 was provided online, and did not include a demonstration of the employee's ability to perform CPR.
