Based on record review and interview, the manager failed to ensure that the caregiver or assistant caregiver documented the services provided in a resident’s medical record according to the resident’s service plan for one out of two residents sampled. The deficient practice posed a risk as services could not be verified as provided against a service plan. Findings include: 1. A review of R1’s medical record revealed a service plan that included the following: Bath, two times a week and as needed; and Brush dentures, daily and as needed. 2. A review of R1’s November 2025 activities of daily living sheet revealed no documentation of the following: Bathing documented as completed once a week instead of two times a week from November 1-30, 2025; and Brushing of dentures documented as completed once a week instead of daily from November 1-30, 2025. 3. In an exit interview, the findings were reviewed with E1 and no additional information was provided.
