A relicensure survey was completed on 12/3/24. A deficiency was cited. Based on observation and interview, the residence failed to ensure the grounds of the residence were maintained to protect residents from hazards, affecting eight current residents.Findings include:On 12/3/24 at 11:26 a.m., during an environmental tour of the residence, the backyard concrete ramp that led from the house to the backyard garden had approximately a four-inch drop-off from the pavement to a gravel path on the north side with an exposed sprinkler hose on the ground. Along the gravel walking path in the front yard of the residence, the metal edging had become uplifted and protruded up from the ground, creating potential tripping hazards.On 12/3/24 at approximately 3:24 p.m., the owner stated that the drop-off along the back patio ramp was created by the last snowstorm and was unaware that the metal edging stuck up along the gravel walking path. She reported that it needed to be fixed and was a potential tripping hazard.
