A relicensure survey was completed on 7/30/24. A deficiency was cited. Based on interview and record review, the residence failed to maintain a personnel file for each of its employees, for one of three sample staff (#3), affecting seven current residents.Findings include:Review of the personnel file for Staff #3 revealed she was hired on 4/23/24, and began working at the residence on 5/1/24. However, Staff #3' s file contained no documentation of orientation and training. On 7/30/24 at 1:58 p.m., the administrator designee stated Staff #3 had completed her orientation and training at a sister residence and it was left there. The administrator designee acknowledged that orientation and training needed to be maintained in Staff #3' s personnel file. On 7/30/24 at 3:00 p.m., the administrator designee stated she expected Staff #3' s documentation of orientation and training to have been maintained in her personnel file onsite. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised it must review and maintain the following processes in accordance with existing program regulations found at 6 CCR 1011-1, Chapter 7.14.21 The assisted living residence shall be responsible for complying with authorized practitioner orders associated with medication administration except for those medications which a resident self-administers.
