The facility failed to have documented evidence that direct care staff had completed all required continuing education.

The facility failed to conduct required fire drills and document fire drills on the required form.

The facility failed to review the plan at least annually to reflect changes in information, within 30 days following a disaster, within 30 days after a drill, and within 30 days after a change in rule or policy.

The facility failed to provide a written contract with a fire alarm firm to perform inspections, testing, and system maintenance at least every six months.

The facility failed to ensure interior walls and/or ceilings were constructed with a material having at least a 20-minute fire rating, or at least 3/8 gypsum board.

The facility failed to provide the required sprinkler system to meet NFPA 13, 13D, 13R, or meet the requirements of Chapter 33.

The facility failed to provide metal or other approved waste containers for kitchens, janitor closets, laundries, mechanical or boiler rooms, general storage, and similar places.

The facility failed to provide residents and residents' legally authorized representative with a copy of the plan upon admission, on request, and when a significant change to the plan is made.
