Questions for Your Tour

  • 1Given the facility's large size, what specific systems are in place to ensure consistent communication between the administration and family members regarding a resident's daily needs?
  • 2I noticed the physical therapy team is highly regarded here; how do you integrate that level of specialized care into the daily routine for residents who need extra support with mobility?
  • 3With the current staffing levels, what is the protocol for ensuring that personal care and hygiene needs are met promptly throughout the day and night?
  • 4How does the facility safeguard personal belongings and clothing to ensure that items remain accounted for as residents transition through their daily activities?
  • 5Since medication management is a priority for us, could you walk us through the process for administering and tracking medications to ensure accuracy and safety?
  • 6I see that you actively engage with feedback online; how do you use that input to improve the dining experience and overall quality of life for your residents?

Key Review Excerpts

The therapists were AWESOME, the head nurse Julie, was AWESOME, the RN's, the LPN's, the CNA's, everyone was great.

Rehab family member · 2023★★★★★

She hit call button and waited 4 hours before anyone came to change her after she souled herself. Was told button was broke!!!! Really???? What if she fell?????

Granddaughter of resident · 2024☆☆☆☆

The nurses.... are practically non existent. If you are lucky enough to find one, they push the work onto an aide, even when they are not busy! The nurses lack a good attitude.

Sister of rehab patient · 2024★★★☆☆

3 complaint-triggered

This facility shows recurring problems across multiple surveys, with families filing three complaint reports about issues including pressure ulcer care and respiratory care. The main problem areas are fire safety systems, electrical systems, and food service management, with many deficiencies repeating across different inspections. While most issues are marked as corrected, the pattern of repeated violations and one uncorrected construction deficiency suggest ongoing operational challenges families should investigate further.
