Documentation issues regarding client bowel movements and medication administration errors.
Unauthorized locking of a closet door.
Missing oxygen signage, unsafe water temperatures, and home maintenance hazards (flooring/tub).

Facility submitted a Plan of Correction for most, but not all, cited deficiencies on page 11-13.; Plan of Correction submitted by Volunteers of America Western Washington addressing citations related to Physical/Safety, IISP development, and Restrictive Procedures.
Failed to ensure national fingerprint background check for one staff member (Staff A) hired after 2016.
Unsafe living conditions (Client 3); lack of oxygen-safety postings (Clients 6 and 7); water temperatures exceeding 120F (Client 6).
Issues identified regarding oxygen signage, water heater temperatures, carpet tripping hazards in Client 3's home, and required flooring/tub repairs.
Failed to document completion of Continuing Education (CE) requirements for four staff members (A, B, G, and H).
Failure to implement nighttime repositioning schedule for Client 6 as per support plan.
Deficiencies in IISP documentation, bowel movement protocols, and repositioning tracking.
Bowel protocols not tracked or implemented for Clients 6 and 8; medication error for Client 7 handled without consulting a professional.
Unauthorized restrictive procedures (locked closet, door alarms) implemented for Client 5 without documentation or behavioral assessment.
Unauthorized use of a locked closet in Client 5's home.
