The following deficiencies were found during the on-site compliance inspection conducted on August 15, 2024:

When medication is stored by an assisted living facility, a manager shall ensure that:R9-10-816.F.1

Based on observation and interview, the manager failed to ensure medications stored by the facility were stored in a locked area. The deficient practice posed a health and safety risk to residents with access to the medications. Findings include: 1. During the environmental inspection of the facility, the Compliance Officer observed an unlocked medication cabinet in the common area. The medication cabinet contained medication for ten residents. 2. In an interview, E1 acknowledged the aforementioned medications were not stored in a locked area and were accessible to residents.

Based on observation and interview, the manager failed to ensure poisonous or toxic materials stored by the facility were stored in a locked area and inaccessible to residents. The deficient practice posed a health and safety risk to residents with access to the poisonous or toxic materials. Findings include: 1. During the environmental inspection of the facility, the Compliance Officer observed "Christy's Red Hot Blue Glue", "Restorox Clean, Kills, and prevents Mold & Mildew", six cans of "Spray Paint" two one gallon "Paint" and "Ortho Home Defense" stored in the maintance closet. The maintance closet door had a locking device installed, but it was unlocked at the time of the observation. 2. In an interview, E1 acknowledged the aforementioned poisonous or toxic materials were not stored in a locked location and accessible to residents.
