Based on record review and interview, the manager failed to ensure a resident's medical record contained documentation of notification of the resident of the availability of vaccination influenza (flu) and pneumonia were offered every 12 months, for one of two residents sampled. Findings include: 1. A review of R2's (admitted April 2021) medical record revealed documentation of notification of the availability of the flu vaccine in 2022. However, evidence of documentation of the availability of the pneumonia vaccine was offered or received was unavailable for review. 2. In an interview E1 acknowledged R2's medical record did not contain evidence of documentation of the availability of the pneumonia vaccine being offered annually.

Based on record review and interview, the manager failed to ensure medication administered to a resident is administered in compliance with a medication order for one of two residents sampled. Findings include: 1. A review of R2's medical record revealed a service plan which indicated R2 received directed care services and administration of medication. The medical record contained a doctor's order, dated August 2, 2023, directing R2 take "KCL 20 meq po QD." 2. A review of R2's Medication Administration Record (MAR) for August 2023 revealed a section documenting the administration of "Potassium CL ER 20 MEQ TABL, Take 1 Tablet by Mouth Daily." The section contained documentation indicating the medication was administered to R2 daily, from August 9, 2023, through August 15, 2023, however the record indicated R2 was not administered the medication on August 3, 2023 through August 8, 2023. 3. In an interview E1 acknowledged R2 did not receive their medication as ordered.
