Staff removed the television following an incident of inappropriate behavior, using it as an incentive for the resident to 'act proper' rather than following established rights protocols.

The facility failed to ensure that one of three sampled residents had completed required tuberculosis (TB) testing upon admission. Specifically, immunization records for Resident #2 showed no documentation of a TB skin test despite the resident being admitted on 05/14/21.

The facility failed to maintain hot water temperatures at bathroom sinks in 4 of 6 resident bathrooms within the required range of 100 to 116 degrees Fahrenheit. Observations and logs revealed temperatures as high as 128 degrees F and as low as 71 degrees F, posing a significant burn risk to residents.
