No deficiencies were found during the on-site compliance inspection conducted on April 24, 2025:

Jan 18, 2024Complaint

The following deficiency was found during the on-site compliance inspection and investigation of complaint AZ00205012 conducted on January 18, 2024:

When medication is stored by an assisted living facility, a manager shall ensure that:R9-10-816.F.1Corrected Jan 18, 2024

Based on observation and interview, the manager failed to ensure medication stored by an assisted living facility was stored in a separate locked room, closet, cabinet, or self-contained unit used only for medication storage. The deficient practice posed a risk to the physical health and safety of residents with access to the medication. Findings include: 1. During the environmental inspection of the facility, the Compliance Officer observed a medication lock box in the refrigerator in the kitchen. The lock box was bent in a way that made the box unable to be locked. The box had a child safety latch attached. However, the child safety latch did not act as a lock for the box, as it did not require a key, tool, or special knowledge to open. Inside the box, the Compliance Officer observed resident medication. The Compliance Officer observed no personnel member within sight of the refrigerator. 2. In an interview, E2 reported not knowing child safety latches are not considered locks.
