During a fire alarm, not all residents were evacuated to the designated meeting place; only residents of the SCU were evacuated.
Annual medical evaluations for two residents were missing required information regarding immunizations, chest X-ray results, and mobility needs.
The medication cart contained expired medication and a medication for a resident without an available physician's order.
