The facility failed to maintain hot water temperatures at all fixtures between 100 and 116 degrees Fahrenheit. Specifically, 14 out of 14 tested fixtures, including sinks, showers, and tubs in both resident and common bathrooms, recorded temperatures ranging from 117.6 to 131.2 degrees Fahrenheit.

The facility failed to maintain hot water temperatures between 100 degrees Fahrenheit and 120 degrees Fahrenheit. During the inspection, 14 hot water fixtures, including sinks in common bathrooms and resident bathrooms, as well as shower and bathtub fixtures, were found to have temperatures ranging from 117.6 degrees F to 131.2 degrees F.

The facility failed to ensure that all staff members had no substantiated findings listed on the North Carolina Health Care Personnel Registry. Specifically, one sampled staff member was found to have a documented substantiated finding of fraud to a resident on the registry. Additionally, the facility failed to perform required registry checks for another sampled staff member prior to hire.
