What this means for your family

While some families report positive rehab outcomes, the high frequency of reports regarding neglect, unsanitary conditions, and unresponsive staff is deeply concerning. Key Review Excerpts

My wife fell in the bathroom while transferring back to her wheelchair and nobody responded to the alarm for 20 minutes. On a separate day, a nurse manager somehow decided it was appropriate to humiliate her by making accusations I will not repeat in the cafeteria at lunch time.

Spouse of resident · 2024☆☆☆☆

My father was there for only 1 month and had 2 bladder infections, pneumonia, RSV, norovirus, and fell twice. On one of my visits, the aid transferred my father without a belt and dropped him on the floor right in front of me.

Daughter of resident · 2025☆☆☆☆

The staff was amazing and always tended to my dad’s needs. If there was ever an issue they always notified me. My mom and I visited him everyday and always got our questions answered.

4 complaint-triggered

This facility has concerning patterns of recurring deficiencies across multiple surveys, with families filing four complaints that triggered federal investigations. The most persistent issues involve medication management, infection control, and resident rights, with many problems appearing repeatedly across 2023-2025 surveys. While the facility corrects issues when cited, the pattern of recurring violations in critical care areas like pharmacy services and infection prevention suggests ongoing operational challenges that families should discuss thoroughly during visits.
