What this means for your family

While the facility has a history of providing effective physical therapy, the recent and consistent reports of neglect, hygiene failures, and delayed responses to call lights are significant red flags. If you are considering this facility, we strongly recommend conducting unannounced visits during weekends or evenings to observe staffing levels and response times firsthand.

Key Review Excerpts

My mother laid in her soiled depends for lengths of time before she was cleaned up. She was dehydrating and I asked for the glass for her to have water on a Thursday because hers had disappeared.

Long-term resident's family · 2024☆☆☆☆

She was so heavily sedated she could barely move, couldn’t speak, and had to use hand gestures to communicate. I later learned they had her on sedation medication that was supposed to be given only as needed.

Memory care family member · 2025☆☆☆☆

The staff mean well, but they are so understaffed -- especially with nurses. Wounds are not being checked for days. My mother-in-law is in the same clothes she arrived in three days ago even though we provided plenty of clothing.

The Emeralds at Faribault has concerning patterns of deficiencies involving protection from abuse and neglect, with families filing multiple complaints triggering investigations. Recurring issues include resident care quality, medication management, and infection control across the three-year period. While the facility reports correcting all violations, the repeated nature of problems—particularly abuse prevention deficiencies appearing in 2024 and 2025—suggests ongoing challenges requiring careful evaluation during any visit.
