Key Review Excerpts

Upon arrival that cold day we were greeted by warm smiles and caring humans. As hard as it was for me, they made me feel at ease leaving her.

The staff is AMAZING all the way from the administrative side to all the floor staff like nurses, maintenance, cooks. Everyone is friendly and really go out of their way to make the residents happy.

Families filed a complaint that led to findings about accident prevention and supervision in 2024. The Elms Center has recurring issues with infection control programs and fire safety systems across all three surveys, along with problems in medication management and nursing staffing. While all deficiencies show correction dates, the persistence of infection control and safety issues across multiple years suggests ongoing operational challenges that families should discuss during visits.
