The facility failed to maintain clean and well-repaired walls, ceilings, and floors. Observations included broken and protruding tiles in a common shower room, mold and mildew buildup in various bathrooms, and accumulated dirt and grime on floors and surfaces. Additionally, several resident refrigerators were found to be dirty and unhygienic.

Specific issues included mold, mildew, and dirt buildup in showers, cracked or protruding tiles, and soiled resident refrigerators. Additionally, light fixture covers throughout the halls were found to be split, jagged, and cracked.
