The facility failed to complete a criminal background check for one of three sampled staff members. The Administrator could not produce the personnel record to demonstrate that the required background check results were available in the staff person's file.

The facility failed to maintain clean and well-repaired walls, floors, and fixtures in several common bath areas and resident rooms. Specific issues included mold, mildew, and dirt buildup in showers, cracked or protruding tiles, and soiled resident refrigerators. Additionally, light fixture covers throughout the halls were found to be split, jagged, and cracked.

The facility failed to maintain clean and well-repaired walls, ceilings, and floors. Observations included broken and protruding tiles in a common shower room, mold and mildew buildup in various bathrooms, and accumulated dirt and grime on floors and surfaces. Additionally, several resident refrigerators were found to be dirty and unhygienic.

The facility failed to obtain an ordered lab value for a resident and failed to follow up with the physician regarding the results. Specifically, there was no documentation that a Depakote level was rechecked or that the physician was notified of the results as ordered.

The facility failed to ensure follow-up for a resident's acute health care needs by failing to obtain an ordered lab value for Depakote levels and failing to follow up with the physician regarding the results. Consequently, there was no documentation that the required follow-up lab work was performed or that the physician was notified of the previous high lab results.

The facility failed to assure physician notification for a resident regarding the refusal of antibiotics and incontinence care. This failure to provide necessary follow-up resulted in the resident developing septicemia and death.

The facility failed to ensure a resident received care and services that were adequate, appropriate, and in compliance with relevant laws and regulations. Specifically, the facility failed to ensure proper health care referral and follow-up following medication and care refusals.

The facility failed to ensure physician notification for a resident regarding the refusal of antibiotics and incontinence care. This failure resulted in the resident developing septicemia and subsequently passing away.
