Key Review Excerpts
“We walked in to Broadway and could hear my grandmother screaming from the hall. She was half on and half off the bed. She had clearly been screaming for a little while as she was very red in the face. The nurse met us in the room and said, “she heard her screaming, but was going to help her when she finished picking up dishes from lunch.””
“I was disappointed when I walked into my mom's room and found her trying to find her mouth with her spoon. Her food was placed in front of her and she was left alone to feed herself. My mom has swallowing and choking problems and she should never be left alone when eating.”
“After spending eight days together during the evacuation of The Broadway from Lockport to Abbeville for Hurricane Ida, I can honestly say that the Broadway team demonstrated grace under pressure, and complete love and respect for their residents.”

Inspection History
Medicare Inspection History
3-year lookback · Medicare 2026
This facility has a concerning pattern of complaint-triggered deficiencies with nearly half (17 of 36) resulting from families filing reports about problems. The most recurring issues involve infection control appearing across multiple years, medication management with error rate problems, and resident protection from abuse and neglect. While the facility has correction plans for most deficiencies, the persistent complaints and repeat violations in critical areas like infection control suggest ongoing quality concerns families should investigate thoroughly before considering placement.
