Specifically, for residents requiring low salt and 1800 diabetic diets, there was no corresponding menu available to guide food service staff. Observations also noted the presence of a salt shaker on the kitchen table and a lack of a regular diet menu for the current week.

The facility failed to ensure that therapeutic or modified menus were available to guide food service staff for three residents requiring specific diets. Specifically, residents on low salt and 1800-calorie diabetic diets were not provided with corresponding menus, and a salt shaker was observed on the kitchen table during meal service.

The facility failed to conduct and document required fire drills, with no documentation of a fire drill performed in 2025. During an observation, a resident with cognitive impairment failed to evacuate when the fire alarm was activated. Additionally, the resident was unaware of what a fire drill was and did not recall hearing the alarm.
