Based on observation and interview, the governing authority failed to ensure that the Department was notified when there was a change in the manager. Findings include: 1. During the environmental inspection of the facility, the Compliance Officer observed E3’s manager's certificate posted within the facility. 2. In an interview, E1 reported that E1 took over as the facility's manager in May 2025, and E1 contacted the NCIA Board and didn't know that E1 had to contact the Department as well. 3. In an exit interview, the findings were reviewed with E1, and no additional information was provided.
